NCAA Disclosure Forms: NCAA Disclosure Form answers will be reviewed by Cornell College. Affirmative answers or failure to disclose information or providing inaccurate information may result in the creation of an incident report and an investigation through the Cornell College Student Conduct process that could result in disciplinary action.
