Admitted Students Deposit & Notification Form.

Please make the selection that best represents your current status and college decision.:  I am attending Cornell College and will be submitting my $300 enrollment deposit before May 1, 2016, 11:59pm PST.  
 Unfortunately, I will be declining Cornell College’s offer of admission.  
 I need to request an extension to the May, 1 deadline. I understand that Cornell College is not required to extend its deadline beyond May 1, but I am requesting the extension for the reason(s) specified below: Upon submission, our staff will be in contact to discuss your request.  

If you are declining Cornell College's offer of admission, please state the reason(s):  Cornell College is my first choice but there were concerns. I authorize a representative to contact me if the college is in a position to review my scholarship or financial aid status, even if it is after the May, 1 National Candidate's Reply Date.  
 I am not attending Cornell this year, but please add me to your list of prospective transfer students.
