Student interested in starting the process of establishing academic accommodations should schedule an intake meeting with the Coordinator of Disability Services  and may complete the Application for Accommodations form prior to the intake meeting.   Bring any documentation to that meeting or send it ahead of time via email (scanned attachment) , fax or postal carrier.

Request Faculty Notification Letter

Application for Accommodations  This is the initial form for current students requesting accommodations for the first time.  Admitted students should fill out the Application for Disability Accommodations located within their Enrollment Checklist.  

Report something that is inaccessible on campus (physical inaccessibility)
