There are no plan design changes for all benefit plans. Dental insurance is the only plan with a premium increase.
Those who wish to continue participation in the Health Savings &/or Flexible Spending plans must complete new 2012 election forms.
There is a special opportunity this year to enroll in the supplemental life insurance without going through underwriting. The maximum guaranteed issue without underwriting is 7x salary or $200,000 for yourself and $20,000 for your spouse. The new maximum with underwriting is $500,000 for yourself and $250,000 for your spouse. If you do not purchase the maximum amount of coverage, you may add $40,000 each year with no underwriting in future years. Health Insurance
No changes to plan design or premiums for 2012.
Important Notes:
- There are two health insurance plan options: regular Health Care Plan and High Deductible Health Care Plan
- Dependent children may be covered to age 26 under both plans
- If the dependent is aged 19 to 26 and has a health insurance coverage option through their own employer, they cannot be covered under the regular Health Care Plan, but may still be covered under the High Deductible Health Care Plan
- No lifetime plan maximum
- No pre-existing condition limitations for dependents under the age 19
Highlights of the High Deductible Plan:
- Plan requires a deductible of $2,500 per person; $5,000 per family
- Preventative care is payable at 100%, not subject to deductible
- After deductible is met covered services are paid at 100% for the rest of the calendar year (no co-payments, no co-insurance)
Plan design differences:
- Preventive and Well-Baby Care covered at 100% under the HDHP with no calendar year limit - regardless if deductible is met
- No 4th quarter (Oct - Dec) carry-over deductible provision under the HDHP
- Colonoscopies will be covered at 100% if marked as preventative, if marked with a diagnosis code it will be subject to the deductible under the HDHP

Health Savings Account

- Incentive to Participate: Cornell College will deposit $25 each payroll in 2012 into your HSA account

Flexible Spending Accounts
No changes to plan design for 2012.
Important notes:
- You can no longer submit claims for over-the-counter medications unless prescribed by a physician (effective 1/1/11)
- If you are planning to participate in a Health Savings Account (or your spouse participates with his/her employer) and want to participate in a flex spending account, you can only enroll in a "Limited Purpose Flex" account to allow reimbursement of eligible dental and vision expenses only. Health care expenses are to be reimbursed via your Health Savings Account.
- If your spouse is participating in a medical flexible spending account with their employer, you cannot enroll in a Health Savings Account
