Forms for Deposited Students

Students that have submitted their enrollment deposit to Cornell will be sent a check list in early June directing them to complete all or some of the following forms.  If you are unsure of what forms you need to complete, please contact of Office of Financial Assistance.

Cornell Institutional Application - Entering Students: This application must be printed and returned to Cornell.

Cornell Student Employment Application: This application must be printed and returned to Cornell.

Cornell Outside Scholarship Form: This form must be printed and completed ONLY if you are receiving outside scholarships.

Federal Direct Loan Entrance Interview: The entrance interview is completed online. Please click on this link to begin your entrance interview.

Dependent Student Federal Verification Worksheet: This form is for DEPENDENT students selected for verification and must be printed and returned to Cornell.

Independent Student Federal Verification Worksheet
: This form is for INDEPENDENT students selected for verification and must be printed and returned to Cornell.

Perkins, McElroy, or Sherman Loan Information Sheet: This form must be printed and returned to Cornell.

William D. Ford Direct Loan Program
Entering students borrowing the Federal Direct Stafford Loan for the first time will need to complete the following so Cornell can certify your federal direct loan.
