Syllabus Information for Disabilities Services
You should include a short statement concerning the College's Disabilities Services on the syllabus for each course you teach. This statement should include the information that the student must have appropriate documentation on file with the Coordinator of Academic Support in order to be eligible for accommodation. The student must request accommodation within the first three days of each term in order to be eligible for accommodation. The student and the course instructor should complete a form available here and in the office of the Coordinator of Academic Support that specifies the accommodations required. You may also wish to refer students to the College's website on Disabilities Services.
An example of a statement about academic accommodations that may be included in your syllabus follows. You are welcome to copy and paste this statement into your syllabus, or you may wish to phrase your statement differently. Please always refer students to the Cornell disabilities services website, and always remind students of their obligation to request accommodation within the first three days of each new term.
Cornell College is committed to providing equal educational opportunities to all students. If you have a documented learning disability and will need any accommodation in this course, you must request the accommodation(s) from [the instructor of the course] as early as possible and no later than the third day of the term. Additional information about the policies and procedures for accommodation of learning disabilities is available on the Cornell web site at http://www.cornellcollege.edu/academic_affairs/disabilities/.
An additional statement would be welcome that states that you are available for students to discuss academic challenges they may be facing so that even students without disabilities will be reminded that you are accessible and available for consultation on academic skills.
